A real check-in,
not a personality quiz.

These are the actual screening instruments used in clinics worldwide, scored the way the research defines. Answer honestly and you get a score, what that range typically indicates, and a straight answer about what to do next.

Before you start. These are screening tools, not diagnoses — only a licensed clinician can diagnose a mental health condition, and screeners can be wrong in both directions. Your answers never leave your device: there is no account, no email capture, no analytics, and nothing is sent to us or stored anywhere. Close the tab and it is gone.

Tip: press 1–5 to answer, Backspace to go back.

What this is and isn't. A screening indicator — a signal that something may be worth a proper look by a professional. It is not a diagnosis, it does not account for your history or circumstances, and a low score does not mean nothing is wrong. If any of this does not match your lived experience, trust yourself and talk to someone qualified.

One score is a dot. Ten is a line.

A single result tells you about today. Tracking the same screener over weeks tells you whether things are actually moving — which is the part that matters, and the part Mind Matter is built to do.

Track this in Mind Matter

Mind Matter includes 18 validated screeners, mood tracking, journaling and self-guided CBT and DBT programs.

About these instruments

Why only these three?

The app ships eighteen validated screeners. Only three appear on this public page — the ones whose licences unambiguously permit free commercial use. The rest are free for clinical and research use but have not been cleared for a commercial website, so they stay in the app rather than being reproduced here.

If any of this brought something up

Thinking honestly about how you have been can surface more than you expected. If you are struggling right now, you do not have to wait until you have finished a questionnaire.
